a better understanding of supported living structures, their funding,
your role, & how to improve your adult son or daughters
outcomes.
Bring your laptops and leave with several tools that will be demonsstrated
and provided free as part of this workshop. Time tested spreadsheets,
documents, and other tools that help to streamline otherwise complex
tasks are shared to increase your success.

individual needs, questions and concerns. Attending a training is
encouraged so that the majority of questions may be answered. This
time may then be spent addressing specific issues.

Service Design Template
A tool that will save you dozens of hours and
is said to be "an amazing piece of work" and "a
timesaver for a potential provider".

The template can save you months of research and work. Writing
the Service Design is one of the most daunting tasks in starting
your Supported Living Agency. The Service Design Template holds
all the areas that need to be included by most regional centers,
though it is geared toward San Andreas Regional Center.
Some revisions or additions for the regional center in your area
may be required. You will want to obtain your regional center's
requirements before you start!
Many of the areas are populated with information that there is
no reason to rewrite; others you are prompted with specific questions
that need to be completed to that fit the criteria for that area.
Examples are offered throughout and are there for modification and
adaptation to your agency and son/daughter.
This tool may be purchased for $129.00 from Journey of Choice,
Inc. for the sole purpose of a single use by the purchaser to create
a service design for a supported living agency using Title 17 as
the guide.

Training at Your Site (California Only)
The "work"shop is an overview of starting a supported
living and the in's and out's of doing so.
This workshop is for those that are seriously considering this
as an option and are ready to acquire the tools they need to start.
Parents are capable of anything, we run businesses, households,
and providing supported living services for their adult son or daughter
is no exception. This "how-to" workshop is open to both
parents and non-parents interested in establishing person-centered
supported living services.
You will learn about the steps to starting, the funding sources,
the funding needed, staffing tips and leave with tools that will
increase your success. We will review and practice some of the tools
that will make your job easier.
Participants leave with a CD that contains time-tested and refined
documents, spreadsheets that help you to do your billing, payroll
and other tasks. These documents may be altered to meet the needs
of your newly formed agency.
Participants are encouraged to bring their laptop with a working
CD drive. Microsoft Office is needed to open most of the documents
or Open Office if you don't have Microsoft Office, which is available
at www.openoffice.org.
The Service Design Template is not included but will be made
available to attendees interested. A 5 minute demo is offered at
each event.
